There may be legal requirements in your state as to what needs to be disclosed, so you may want to consult an attorney, but in general, you're looking to provide information regarding the size of the edition, the materials used, etc. Here's a rough draft that may be useful:
CERTIFICATE OF AUTHENTICITY
This is a fine art giclee reproduction of an original [insert art medium] entitled [insert painting title] by [insert artist’s name].
This [insert print dimensions] print is number ____ out of an edition of ____ prints. Additionally, there are ___ artist proofs, and ____ printer proofs in this edition.
[Insert painting title] is also available in an edition of ____ prints measuring [insert other print size dimensions if applicable]. No other editions are in print.
This giclee reproduction was made by [insert printer’s name] of [insert printer’s location] using archival-quality materials to ensure long lasting beauty and to protect your investment in fine art. This giclee is printed on [insert type of media used] using archival, pigmented inks. This print was reviewed by the artist and printer to assure its fidelity to the original artwork.
