I purchased a book a few years ago called Legal Guide for the Visual Artist by Tad Crawford. Mine is the fourth edition (2001) and I don't know if it has been updated since then. It is geared towards fine artists, illustrators, computer artists, graphic designers and photographers.
Anyway, I pulled it out to see if this might be of help, and I think it may. When I went to the index to look up forms, I counted 48. Besides an Artist-Agent Agreement, there is an Assignment Form - Photographer's Model, Copyright Assignment Form, Bill of Sale, Invoice, Permission and Print Contract with Gallery to name a few. Besides the forms, this is a great book. I have referred to it several times. Tad Crawford is an attorney, author and prominent advocate for artists' rights. I was also surprised to find that it is not too difficult to understand. It's great for artists either starting out or mature in their career.
